Two combined clinics have begun as part of the Connecting Care for Complex Chronic Disease project at the Logan Healthcare Centre at Griffith University, for Logan Hospital patients.
The two clinics, Cardiology/Diabetes and Renal/Diabetes, are already making a difference to patients like Christina Lehne.
Ms Lehne said it was wonderful not to have to attend multiple appointments.
Clinical lead for the project Dr Tony Russell said the clinics operated with GPs, community staff, and hospital staff working together in an integrated care model to provide best patient care for patients with complex chronic disease.
“This is a holistic model of care for patients of Logan Hospital and provides an alternative to the current patient management framework.”
